University Interscholastic League Forms:
The links on this page refer to forms that the University Interscholastic League requires student athletes and their parents to complete before they are allowed to participate in junior high and high school athletics. All of these forms and manuals can be found on on the Uvalde CISD Athletic website or on the UIL website under athletics.
Medical History: Medical History must be completed annually by parent (or guardian) and student in order for the student to participate in athletic activities. This form must be on file prior to participation in any practice, scrimmage or contest before, during or after school. (Included in UCISD packet).
Physicals: The Participation Physical Evaluation (Physical Examination) must be completed by a Physician, a Physician Assistant licensed by a State Board of Physician Assistant Examiners, a Registered Nurse recognized as an Advanced Practice Nurse by the Board of Nurse Examiners or a Doctor of Chiropractic. Uvalde CISD requires athletes to have a new doctor's physical every year they participate in athletics.
Acknowledgement of Rules: This form must be signed yearly by both the student athlete and parent/guardian and be on file at your school before the student may participate in any practice session, scrimmage, or contest.
Concussion Acknowledgement Form: The UIL has created this Concussion Acknowledgement Form, which will be required for all student athletes in grades 7-12.
UIL Parent Information Manual: UCISD Athletics is required to provide you access to the UIL Parent Information Manual. If need a hard copy of this manual you may request one from the Uvalde Athletic Office. You must read and agree with this entire document.
Character Attitude Responsibility Ethics: Sportsmanship and UIL Athletics: The UIL is dedicated to offering character-building, educational competition to member school students in Texas. In an attempt to give appropriate athletic competition to our student-athletes, it is imperative that we have a policy in place addressing the correct and incorrect way to conduct oneself as it pertains to UIL sanctioned events, whether it be in practice, competition or outside the athletic arena.
Previous Athletic Participation Form: All students in grades 9-12 who have ever practiced or participated in any UIL activity in grades 8-12 at another school must have this form completed by the previous school (last school of participation) and be approved by the District Executive Committee before they are eligible to participate at the varsity level at the new school.
